- You do not have to be ready to quit before you make drinking safer. Harm reduction is a federally recognized, evidence-based framework, and it applies to alcohol just as it does to opioids.1
- Start with measurement. A standard drink is 14 grams of pure alcohol: a 12 ounce regular beer, a 5 ounce glass of wine, or 1.5 ounces of spirits. Most heavy pours are two standard drinks or more.2
- The highest-impact safety moves cost nothing: count your drinks honestly, space them out, eat before and while drinking, and never drive. Never combine alcohol with benzodiazepines, opioids, or sleep medications; the combination suppresses breathing and is a common pattern in overdose deaths.312
- Naltrexone reduces heavy drinking days and does not require abstinence first. Targeted dosing, taking it before drinking, has trial support and fits a harm reduction plan.456
- Warning signs that the plan is failing: morning shakes, drinking to stop withdrawal symptoms, or blackouts becoming more frequent. If your body is physically dependent, do not stop abruptly on your own. Alcohol withdrawal can cause seizures and can be fatal; reduction and cessation at that stage need clinician supervision.1011
Most advice for people who drink heavily starts from the assumption that the goal is quitting. If you are not there, that advice reads as noise, and you stop listening. This article starts from a different place: you are drinking heavily, you are not pursuing abstinence right now, and you would still prefer not to wreck your health, your license, or your relationships in the meantime. There is a real evidence base for exactly that position. One boundary up front: harm reduction lowers risk. It does not make heavy drinking safe, and no honest version of it claims to.
Harm reduction is an evidence-based framework, and it includes alcohol
Harm reduction has a public reputation built on needle exchanges and naloxone, which leads many people who drink to assume the framework has nothing to do with them. It does. SAMHSA's Harm Reduction Framework defines harm reduction as an evidence-based approach that engages people who use substances where they are, reduces overdose and health harms, and offers low-barrier access to care, without making abstinence the price of admission.1
Three ideas from that framework matter for a person drinking heavily:
- Reducing harm is a legitimate goal in its own right. Fewer heavy days, safer patterns, and fewer medical consequences count as real progress even if total consumption never reaches zero.
- Low-barrier beats no engagement. A plan you will actually follow at eight drinks a night beats an abstinence plan you will abandon by Friday.
- Harm reduction and treatment sit on the same continuum. Starting with safer-use tactics does not lock you out of medication, therapy, or abstinence later. The evidence runs the other way: people engaged in harm reduction services enter formal treatment at higher rates, not lower.1

Step one: know what a drink actually is
You cannot reduce what you have not measured, and almost nobody who drinks heavily measures. The NIAAA (National Institute on Alcohol Abuse and Alcoholism, the federal research institute for alcohol) defines a standard drink as 14 grams of pure alcohol. In practice:2
- 12 ounces of regular beer at about 5 percent alcohol
- 5 ounces of wine at about 12 percent alcohol
- 1.5 ounces of distilled spirits at about 40 percent alcohol
A pint of 8 percent IPA is roughly two standard drinks. A generous home pour of wine is often two, and a tall mixed drink made without a jigger can be three. When people report "four drinks a night," the measured reality is frequently six to eight.
NIAAA also publishes low-risk thresholds: for men, no more than 4 drinks on any day and 14 per week; for women, no more than 3 on any day and 7 per week. Above those lines, the research classifies drinking as heavy or at-risk.23
Use these numbers as instruments, the way you would use a scale or a blood pressure cuff. They locate your current pattern on a map and measure whether anything you try is working. A drop from 60 drinks a week to 35 leaves you far above the low-risk line and still represents a large, health-relevant reduction in exposure. Both facts are true at once, and the numbers let you see both.
Step two: track before you change anything
For one or two weeks, count every standard drink, every day, converting pours honestly. Note the time you started, the time you stopped, and the context. A notes app is enough. NIAAA's Rethinking Drinking program is built around exactly this kind of self-assessment and tracking.3
Two things reliably come out of tracking. First, an honest baseline, usually higher than the estimate you carried in your head. Second, a pattern: the specific days, hours, moods, and settings where the heaviest drinking lives. Risk concentrates in patterns, and you cannot target what you have not seen. Step three: change the pattern, in order of impact
These tactics come from the safer-use logic that runs through harm reduction practice: slow absorption, protect the tasks that require a sober brain, and remove the combinations that kill.
Do not drive. Ever. Impairment starts well below the legal limit and below the level where you feel impaired. Decide how you get home before the first drink. This single rule removes the most immediate way heavy drinking ends a life, yours or someone else's.
Space your drinks. The liver clears roughly one standard drink per hour. Drinks taken faster than that stack, and blood alcohol keeps climbing after you stop. Deliberate spacing, with water or another alcohol-free option between drinks, lowers peak blood alcohol even when the nightly total stays the same. Peak level, not just total, drives blackouts, injuries, and bad decisions.
Eat first, and keep eating. Food in the stomach slows alcohol absorption and flattens the spike. Drinking on an empty stomach turns the same dose into a heavier hit.
Hydrate. Alternating with water reduces dehydration, slows your pace mechanically, and cuts the next-day damage that fuels the next-day drink.
Set the number before the first drink, and set a start time. A limit decided at drink four is not a limit. Deciding sober, in the morning, is the entire trick. Pushing your daily start time later is one of the most measurable single changes available.
Protect your mornings. Structure evenings so drinking ends early enough that sleep and the next morning survive. Waking up needing a drink to feel better is a warning sign covered below.
The rule with no exceptions: do not mix alcohol with other sedatives
Everything above is negotiable and adaptable. This section is neither, and it is where the medical evidence speaks plainly.
Alcohol is a central nervous system depressant. Benzodiazepines (alprazolam, clonazepam, diazepam and related medications), opioids (oxycodone, hydrocodone, heroin, fentanyl and anything contaminated with it), and prescription sleep medications are also central nervous system depressants. Combined, their effects on breathing multiply rather than add. Clinically, the mechanism is respiratory depression: breathing slows or stops during sleep or unconsciousness. Combinations of alcohol with opioids or benzodiazepines appear over and over in overdose deaths, and federal overdose prevention guidance treats mixing depressants as a primary modifiable risk.312
Two practical implications:
- If you are prescribed a benzodiazepine, an opioid, or a sleep medication, tell the prescriber honestly how much you drink. Prescribers can adjust; coroners cannot. NIAAA's guidance flags dozens of common medications that interact dangerously with alcohol.3
- A night of heavy drinking plus "just one" borrowed pill is the specific combination that kills people who never considered themselves at risk of overdose. There is no safe version of it to teach.

Medication that works while you are still drinking
Here is the piece most heavy drinkers have never been told: there is medication designed to reduce drinking that does not require you to quit first.
Naltrexone is an opioid receptor antagonist, a medication that blocks the receptors involved in alcohol's rewarding effect. In the large independent meta-analyses, oral naltrexone reduces heavy drinking days and lowers the risk of returning to heavy drinking, with modest but clinically meaningful effect sizes.47 The VA/DoD clinical practice guideline gives naltrexone its strongest recommendation for alcohol use disorder.6 Acamprosate carries similarly strong evidence, though it is studied mainly as support for people who have already stopped.7
For a harm reduction plan, the most relevant approach is targeted dosing: taking naltrexone roughly an hour before drinking rather than every day. A randomized trial by Heinälä and colleagues found targeted naltrexone reduced heavy drinking without requiring prior detoxification or abstinence.5 This is the protocol popularized as the Sinclair Method, and it deserves a careful look rather than either hype or dismissal; our full review of the Sinclair Method and what the evidence says covers the trial data, the limits, and how to talk to a prescriber about it.
Naltrexone is a prescription medication with real contraindications, including opioid use and significant liver disease, so the conversation runs through a clinician. But the ask is simple and increasingly normal in primary care: "I drink more than I want to. I am not ready to stop completely. I have read that naltrexone can reduce heavy drinking. Red flags: how to know the plan is not working
A harm reduction plan is an experiment, and experiments need failure criteria you define in advance. Watch for these specific signals:
- Morning shakes. Tremor, sweating, or a racing heart in the morning that improves with a drink is withdrawal, and withdrawal in the morning means your body has become physically dependent on alcohol.
- Drinking to stop symptoms. When the function of a drink shifts from wanting to feel good to needing to stop feeling sick, the pattern has crossed a physiological line.
- Blackouts increasing. Occasional memory gaps becoming regular ones means peak blood alcohol levels are climbing despite the plan.
- The numbers refuse to move. You track honestly for six to eight weeks, apply the tactics, and consumption stays flat or rises.
- Consequences are arriving anyway. Liver enzyme results, an ultimatum at home, a scare behind the wheel, missed work.
None of these mean you failed or that harm reduction was the wrong idea. They are data, and they mean the current tools are not matched to the current severity. They also trigger the most important safety rule on this page.
If you are physically dependent, do not stop abruptly
This warning needs stating in clinical terms, because getting it wrong is dangerous. Alcohol belongs to a small class of substances, alongside benzodiazepines, for which withdrawal itself can be life threatening. In a physically dependent person, abrupt cessation or a sharp cut in intake can trigger withdrawal seizures and, in a minority of cases, delirium tremens, a state of profound confusion and autonomic instability with a meaningful mortality rate even with hospital treatment.1011
So the instruction is precise: if you have morning shakes, if you drink to hold off withdrawal symptoms, or if you have had withdrawal symptoms during past attempts to stop, do not quit cold and do not run a steep self-designed cutback. Reduction at that stage is a medical procedure with medication and monitoring, and it can very often be done at home under clinician supervision rather than in a facility. This is also why this article contains no taper schedule. Published withdrawal protocols run through a prescriber because the variables that make a taper safe, prior withdrawal history, liver function, other medications, seizure risk, cannot be assessed from inside your own head.
When to escalate, and what escalation actually means
Escalating does not mean surrendering to a 28-day facility. The evidence-based treatment system for alcohol runs mostly through outpatient care: medication from a primary care doctor or telehealth prescriber, therapy, structured outpatient programs, and mutual-help options that welcome non-abstinence goals. Escalation can be as small as one appointment: a naltrexone conversation, a liver panel, an assessment. Consider it when the red flags above appear, when honest tracking and pattern change get a fair trial without movement, or when the medical picture changes.
One more piece of evidence belongs here, because it answers the fear that keeps many heavy drinkers away from any clinician: the fear that seeking help means being pushed into abstinence forever. Research led by Witkiewitz and colleagues has documented that stable, non-abstinent recovery, drinking at low-risk levels with restored health and function, is a real and durable outcome for a subset of people with alcohol use disorder.89 Recovery, in the current clinical literature, is defined by function and wellbeing rather than by a binary drinking status. A clinician practicing current evidence-based care can work with a reduction goal. If the first one you meet cannot, find a second clinician rather than abandoning the project.
The bottom line
Not being ready to quit is a workable starting position. Measure your drinking in standard drinks and track it honestly. Apply the pattern changes with the biggest safety returns: spacing, food, water, a pre-set number, and an absolute rule against driving. Never mix alcohol with benzodiazepines, opioids, or sleep medications. Ask a prescriber about naltrexone, including targeted dosing, because medication support does not require abstinence. Define your red flags in advance, and treat morning shakes or drinking to stop withdrawal as a hard stop on self-management: at that point, reduction needs clinician supervision, not willpower. The goal this month is less harm. What you want six months from now can stay an open question while you get safer today.
